How does the NDIS application process work?

If you think you’re eligible for the NDIS, there are a few ways you can apply, just choose what works best for you:

  • Visit your local area coordinator (LAC) or early childhood partner. They can explain what evidence you need and help with your application.
  • Download the NDIS Access Request Form (PDF 1MB), fill it out and either email it to enquiries@ndis.gov.au  with your supporting documents (PDF 244KB), or print and mail it to PO Box 700, Canberra, ACT 2601.
  • Call the NDIA on 1800 800 110 to request a form. They can post or email it to you.
  • Visit an NDIS office, ask for a printed form, then return it by email or mail with your supporting documents.

If you send your form directly, the NDIA will share it with your local NDIS partner, who will contact you to go through your application and evidence together.
